At the reception window

Three Eyes is the extra eye on the forehead. Hollow Face is empty sockets with a blank expression. Human Teeth pairs small wide circular eyes with realistic teeth, Creepy Smile pairs bulging eyes and black pupils with a drawn-on grin, and Googly Eyes and Sharp Teeth has mismatched glowing eyes.

In the developed photo

Different Eyes is a changed eye shape, an added eyelid, or a missing pupil in the print while everything else looks right. Different In Photo is the wider version of the same comparison and covers ears, colour and markings as well.

On the camera feed

Black Eyes is the black bar across the face with realistic eyes on it, and it is the only eye entry filed under CCTV. The identifier also offers The Starer as an adjacent check, because that entry is about the patient facing whichever camera you open rather than about its eyes. Worth knowing: the black figure with glowing red eyes that appears on the feed is filed as a hazard rather than a patient, so the identifier never returns it.

How this identifier works

The matching rules are derived from the anomaly list documented on this site, on the full anomaly database page. Each rule reads one published entry and turns the eye part of its sign into a trait: an eye count, a colour, a pupil description, a photo mismatch, or which way the patient faces on camera. Nothing is scored against game files, and no patient data is collected.

Scoring is deliberately blunt. Each entry takes one score from the single rule that matches it, and a trait that is the defining sign for that entry scores higher than one the entry shares with others. Candidates are then sorted by that score. Some entries also carry a contradiction test, so reporting a normal eye colour drops Three Eyes even if you counted three. That is enough to push the obvious answer to the top, and it is not enough to tell you what a patient is not: a blank result only means no rule fired, not that the patient is safe.

The underlying list itself is a reconstruction read from public community write-ups, and the entries name their sources. It is not an in-game audit and it is not complete, so treat the shortlist as a memory aid rather than an answer key.
